inside door handles

Is there a trick to getting the handles off the inside? I removed the set screw but cant get hand off.

The handle unscrews, the shaft has fine threads on it..

Was the setscrew a long one, with an extension on the inside? If so, that extension went through a slot on the end of the shaft that the handle is threaded on.
